- Certificate of Occupancy or Home Occupation Permit information.
- The Charitable Services Business Worksheet.
- Corporate registration, when applicable.
- The IRS determination letter.
- Certificate of Exemption/FR-164 exemption information.
- Tax registration and a Clean Hands certificate.
