The form calls for the legal name, EIN, purpose, offices, registered agent, fundraising methods, other jurisdictions, prior regulatory actions, and relationships among board members and fundraising vendors. The chief executive and chief financial officers certify the filing.

Reconcile professional fundraiser, fundraising counsel, commercial co-venturer, raffle, and campaign records before renewal. The online instructions say outstanding raffle reports and joint financial reports can block completion of the related renewal workflow.

Choose an accepted annual financial report

South Carolina accepts Form 990, 990-EZ, or 990-PF, or the Secretary of State’s Annual Financial Report Form. The online guidance expressly says it does not accept Form 990-N as the state financial report; a postcard filer should complete the state form instead.
